Polaris Teen Center’s official website states that it has closed and ceased all operations. This page is retained as a historical profile, not as an available residential programme or admissions option. The announcement was checked on 13 September 2026.
The provider says it appointed Cariend as its medical-records custodian effective 4 August 2025. That is the stated records-transfer date, not necessarily the date the centre closed. Cariend is not a treatment provider and does not manage medication, billing or referrals for Polaris.
Former patients should use the records-request instructions linked from Polaris’s official notice. The notice advises against sending sensitive personal information by email. Current services, prices, treatment lengths and insurance availability must not be inferred from the former programme.
